Lumbar Epidural and Transforaminal Epidural Steroid Injection Transcription Sample

DESCRIPTION OF PROCEDURE: The patient was brought in to the operating room and laid in the prone position. The patient's back was prepped and draped in the usual sterile fashion. The skin and underlying subcutaneous tissues overlying the L5 and S1 lumbar epidural space was identified and infiltrated with 5 mL of 1% plain lidocaine. A 17 gauge Tuohy needle was advanced through the anesthetized area into the lumbar epidural space under fluoroscopic guidance. Next, 3 mL of Isovue 200 dye was injected, which confirmed spread of the medication in the lumbar epidural space. Next, 4 mL of 0.125% Marcaine with 80 mg of Depo-Medrol was administered. The needle was removed. Attention was then directed towards the right neural foramen at L4 and the skin and underlying subcutaneous tissues overlying this area was infiltrated with 5 mL of 1% plain lidocaine. A 5 inch 22 gauge spinal needle was advanced through the anesthetized area towards the neural foramen at this level. Needle placement was confirmed fluoroscopically and 2 mL of Isovue 200 dye confirmed spread of the medication along the L4 nerve root. Next, 2 mL of 0.25% Marcaine with 40 mg of Depo-Medrol was administered. The needles were removed and bandages were placed over the injection sites. The patient was returned to the supine position and to the recovery room in stable condition. There were no complications as a result of the procedure.
